Contact your health care provider or 1-877- PA HEALTH for more information.

These requirements allow for medical reasons and religious beliefs. If your child is exempt from immunization, he/she may be removed from school during an outbreak.
Please note several immunizations have specific age and interval spacing requirements.  If your child’s immunizations do not meet these requirements, you will be notified and you will have a deadline in which to make corrections.
